Do you accept insurance?

We don’t currently accept insurance. Patients can use FSA/HSA cards or we are happy to provide a superbill for you to submit to insurance for out-of-network benefits.

3-6 months

At this time we are only able to provide services to patients in Arizona. Telehealth options may be available to our Arizona patients once they have established care with their provider.

  • Current medications and supplements list
  • Recent lab results (if available)
  • Completed health history questionnaire (provided before visit)
  • List of current symptoms and health concerns
